Tabitha's Continuum Navigators enhance support for Families

Tabitha, your answer for Elder care, is expanding its reach serving clients and families in need of support. Formerly known as case management, the newly minted Continuum Navigation team has expanded, providing more focused support for clients and families.

The new name better reflects the Tabitha’s mission of compassion, as well as a shift in approach toward a social-based model of service. Continuum navigators are a client’s first stop when exploring health care needs. They partner with clients and families, working together to determine the best options for care at the right time and place.

“This name change and team redesign was made through hard work and dedication to expand and enhance our team’s ability to provide guidance and support to Elders and families,” said Kristine Dykeman-Schoening, Tabitha’s executive director of continuum.

“I appreciate working for Tabitha, an organization that places the value on the Elder first and foremost. By dedicating resources to this new model, we ensure our clients receive the optimal level of service and care,” she explained.

Dykeman-Schoening headed the department redesign alongside Lisa Osborne, director of continuum navigation.

As the only organization in Nebraska able to provide a complete continuum of care—from in-home support, Meals on Wheels and Adult Day services to rehabilitation, skilled nursing, hospice care and everything in between—Tabitha’s continuum navigators are at the heart of its operations throughout 28 southeast Nebraska counties.
